Idempotent upsert of an EvaluationForm with nested questions and criterias. Lookup by guid; creates when not found, updates in place when found. Returns 201 on create, 200 on update, 400 on validation failure. Missing questions or criterias in the payload are preserved (no delete); entries matched by guid are updated, entries without a matching guid are created.
